Hello, is it possible with Dynamic Forms to create a form that looks like the code in this post??

This is the source code, I can't post an image, it basically has three lines, one line for Name:, one line for the input box with JS to put some text in the input box, and some explanation text below the input box.

Working form: www.mountainlakeservices.org/webmaster.html

Yes, this is a pretty easy implementation...

The look and feel features allow you to use the field label and field within the same column, set the colspan and rowspan, hide field label, use question header and footers, set the width width, and so forth...

In this example you would probably utilize the ability to either hide the field label and use the question header as the field label and also use the field footer. You would either check the box to have the field label and field in the same column or use the question header as the label and hide the field label.
