Subject: Creating Tables On A Remote Hosted SQL Server

One of my sites is hosted on a remote sql server. Do you have any tutorials on how to access the database and create a new table  for the Dynamic Forms Module to link to? Any modules for this? do you have the token to get the table after it is created?

Well you would either need to use a software application such as Microsoft Management Studio or Query Analyzer to access the database with the connection string used in your web.config or you would need to use a 3rd party module that handles SQL etc... I believe there are several 'SQL Deluxe' type modules that should handle your needs, you could actually even google 'Creating Table in TSQL' which will show you some SQL statements for generating a table which you can then insert data into directly for form submissions if you wanted to. You would execute this SQL under Host, SQL.
