I've looked through the forums but am still not sure. If I want to charge a one-time setup fee for a subscription, is the best way to do this is to use the trial parameters to set my first month's charge at the subscription+fee rate and then use the regular billing parameters to set it to my normal recurring charge?

Yes, you're on the right track here. Using the subscription trial variables (a1, p1, t1) is the best way to include your set up fee.
Other users may use these trial period variables to break up service payments to let's say 60% as downpayment and 40% for the next.
This would be the same solution for those who want to offer a free trial period, or maybe a discount during the trial period.
