is presented after the label and before the text box. Was this intended? I was expecting the header to be in the line above the question.

Hi. Yes this is working as designed. If you want to do what you are mentioning above you would want to just add a field type of Text/HTML and it would display above the line (depending on the sort order of course).

I tried using the Text/HTML field, but it is left justified on the page in the area reserved for field labels and I wanted it to go across the screen. I ended up using the footer of the prior field and this worked ok. Why would you want to be text between the field label and the text box?

I wonder if a class can be added to the css that would allow you to span text over the label and input control

Good idea. We could add a class tag for any HTML field type. We did find that this was an issue and have corrected it within the latest beta release. Speaking of beta releases we are setting the full release date for Dynamic Registration 2.4 up for mid Februrary. Its been running behind but we have had several enhancements we wanted to make sure are working ok before the full release.
