I implemented the form in version 3.4.5 and found the problem with any dropdown field that won't respond to a mouse click in Firefox 9.0.1 on either XP or Windows7. If you tab through all the fields you can use the keyboard to select but it doesn't respond to a mouse click. I wondered if this was a version issue so upgraded to version 4.x and still doesn't respond to mouse clicks. We're on the lastest DNN version 6.1.2. The problem is the following page: http://www.gaylegoldstone.com/Contact.aspx

We have another form that requires subscription and it also doesn't respond to mouse click in dropdown menu fields. Thanks for any help someone can provide or suggest.

Actually, I needed to add more details to this. None of the fields in the form are responding to mouse clicks, except the first field if you place the mouse cursor a bit above the field. If you can get the first field to take the click, then you can enter in to all fields by clicking the tab key and space bar for dropdown and radio buttons.
